中医气功战“疫”:提升正气铸盾

「导读」新春伊始,面对新冠肺炎的蔓延,一场中西并重、防控结合的疫病阻击战正在如火如荼地进行着。其间,中医药的疗效令人满意:中药复方可改善患者症状、控制病情进展、减轻并发症发生;八段锦、五禽戏等传统中医气功,也出现在湖北武汉一线病房、方舱医院等处,由于其身心并调的特点,受到许多患者的欢迎。

新春伊始,面对新冠肺炎的蔓延,一场中西并重、防控结合的疫病阻击战正在如火如荼地进行着。其间,中医药的疗效令人满意:中药复方可改善患者症状、控制病情进展、减轻并发症发生;八段锦、五禽戏等传统中医气功,也出现在湖北武汉一线病房、方舱医院等处,由于其身心并调的特点,受到许多患者的欢迎。近日,国家卫生健康委员会办公厅、国家中医药管理局办公室颁发的《新型冠状病毒肺炎恢复期中医康复指导建议(试行)》(以下简称“中医康复指导”),也将八段锦、六字诀分别作为“传统功法”和“其他疗法”予以2。由此,传统中医气功战“疫”再次引起了人们的关注。

一、气功战“疫”文献复习

回顾中医气功学发展的历史,可以清楚地看到,其间不乏气功参与疫病防治的经历,无论是对疫病的预防、治疗,还是康复,古代中医气功学家都留下了十分经典的论述和案例。

在预防方面,《内经》的论述影响广泛。《素问·遗篇·刺法论》明确指出:“黄帝曰:余闻五疫之至,皆相梁易,无问大小,病状相似,不施救疗,如何可得不相移易者?岐伯曰:不相染者,正气存内,邪不可干,避其毒气,天牝从来,复得其往,气出于脑,即不邪干。气出于脑,即室先想心如日,欲将入于疫室,先想青气自肝而出,左行于东,化作林木;次想白气自肺而出,右行于西,化作戈甲;次想赤气自心而出,南行于上,化作焰明;次想黑气自肾而出,北行于下,化作水;次想黄气自脾而出,存于中央,化作土。五气护身之毕,以想头上如北斗之煌煌,然后可入于疫室。”中医学界乃至广大百姓耳熟能详的“正气存内,邪不可干”这句话,即是古人针对疫病的预防并结合中医气功提出的。其中的操作方法属于气功学中的存想法,现代所说的冥想与此几近相似。

在治疗方面,最早的记载可能是隋代太医令巢元方主持编写的《诸病原候论》。该书在“时气病诸候·时气候”中,对“病无长少,率相似者”的时气病提出了以肢体运动(调身)为主的导引方:“清旦初起,以左右手交互从头上挽两耳,举;又引鬓发;即面气流通,令头不白,耳不聋。又,摩手掌令热,以摩面从上下二七止。去汗气,令面有光。又,摩手令热,从体上下,名曰干浴。”该书还有针对“温病诸候·温病候”的以存想(调心)为主的导引法:“常以鸡鸣时,存心念四海神名三遍,辟百邪止鬼,令人不病。东海神名阿明,南海神名祝融,西海神名巨乘,北海神名禺强。又云:存念心气赤,肝气青,肺气白,脾气黄,肾气黑,出周其身,又兼辟邪鬼。欲辟却众邪百鬼,常存心为炎火如斗,煌煌光明,则百邪不敢干之。可以入温疫之中。”在“疫疠病诸候·疫疠病候”中也有类似的导引法。

在康复方面,温病大家叶天士则将气功与药物结合,应用于热病(包括疫病)的康复期。《临证指南医案》卷一“虚劳”、卷二“吐血”中有三个堪称经典的病例。如:对“冷痰上升,交子后干咳,肾虚阳不潜伏”的程姓患者,叶氏医嘱“从摄固引导,勿骤进温热燥药”;而对“阳虚生外寒,阴虚生内热”的何姓患者,叶氏提出的治疗方案是,在用药的同时要求患者“潜心涤虑,勿扰情志,再于子午参以静功,俾水火交、阴阳偶,是药饵以外工夫,皆培植生气之助”;对于“精气暗损为病”的陆姓患者,叶氏认为,此“非草木攻涤可却,山林寂静兼用元功,经年按法,使阴阳渐交而生生自振。徒求诸医药,恐未必有当”。叶氏所倡导的药物疗法与气功疗法相结合、医生处方治疗与患者主动锻炼相结合的康复思路,由此可见一斑。

二、气功战“疫”机理概说

中医气功疗法是一种发挥患者自身抗病潜能的自我身心疗法,其长于扶正而逊于驱邪,由此,对疫病预防、治疗、康复的总以培育正气、铸就盾牌为主,而以因势利导、驱邪外出为辅。具体地说,体现在以下几个方面。

1.补益正气,扶正祛邪

气是中医学的核心概念,大家耳熟能详的“人活一口气”正是道出了中医对人体之气的重视。正如国医大师陆广莘所指出的,中医学十分重视人体的正气,以及由其产生的主体性反应,在治疗中善于发现、依靠、调节人的主体性反应,养生、治病都着眼于“疏其血气,令其条达,而致和平”(《素问·至真要大论》),即扶助正气以实现祛除病邪,恢复健康状态。中医气功正是通过自我主动锻炼身形、调节呼吸、调整心理思维与情绪活动,着眼于补益人体的正气,通俗说也就是提升正能量,建立形、气、神之间的正向循环,提高人体的自我抗病、自我修复、自我疗愈、自我调节能力,重在调动人体内在的积极性主动性,到达扶正祛邪,甚至是不战而屈人之兵的作用,最终到达神安、气充、形健,实现“正气存内,邪不可干”的健康目的。八段锦、五禽戏、六字诀等中医气功代表性功法在此次抗击新冠肺炎疫情中的应用,正是基于这一机理,激发人体自身正气,鼓荡患者气血,鼓舞必胜信心,最终战胜疫情。

2.调和阴阳,权衡以平

“和”是人体健康的重要保证,也是健康的标志,包括人与自然的和谐、人体内部功能的和谐。上海市名中医王庆其先生认为“健康是一种‘三和’的状态”健康的本质就是“天人和”(人应该顺应自然界阴阳变化)、“形神和”(做好生理和心理的调摄,保持心身协调)、“气血和”(通过各种措施使人体气血充沛运行有序)。笔者研究气功学中的“和”,得出的观点与王先生不谋而合,发现气功锻炼能够起到顺天辟邪和天人、动静结合和形神、辨证施功和阴阳、调畅气机和内气、相辅相成和脏腑等“和”的作用,这在气功战“疫”中具有不小的意义。以“中医康复指导”2的八段锦为例,其中的第一段“两手托天理三焦”、第三段“调理脾胃需单举”的主要作用是和谐气机的升降;第二段“左右开弓似射雕”(旧称“左肝右肺似射雕)、第五段“摇头摆尾去心火”的作用是和谐脏腑、水火与五行生克。再以其所2的六字诀为例,该功法强调呼气发音,旨在将人体不和之气与从外界吸入的疫病之气“吐”出体外,即古人所说的将“我之气还天地”。

3.疏通经络,因势利导

经络于人体健康和疾病防治的重要性是不言而喻的。李时珍“医不知此,罔探病机;仙不知此,难安炉鼎”之论,尽管讲的是奇经八脉的重要性,但实际也是古代医家对整个经络系统重要性的经典论述。气功,古代也有“导引”之称,在《黄帝内经》时代,“导引按蹻”与“毒药”等并列为当时最主要的五大医疗措施之一,故当代历史学家郭沫若先生说:“古人所说的道(导)引,即今人所说的气功。”导引在中医气功学中,又是一个动词,义近“引导”。气功锻炼时,通过形体的运动(调身)、呼吸的调控(调息)、身心的安静和意念的应用(调心),来导引内气的运行(包括运行的方向和速度),由此充实脏腑之气、活跃经络之气,并以此实现防病治病的目的。具体地说:在未患病或患病的早期施以气功,可奏畅气血、利九窍之效,诚如张仲景之所谓“若人能养慎,不令邪风干忤经络。适中经络,未流传脏腑,即医治之。四肢才觉重滞,即导引,吐纳,针灸,膏摩,勿令九窍闭塞”。金元四大家之一的张从正,还将华佗五禽戏等导引列为汗法而用于临床。这些,对新冠肺炎疑似患者和早期患者的治疗颇有参考价值。在患病以后和康复阶段,科学合理的气功锻炼,可以畅通经络,因势利导驱邪外出,这或许就是“中医康复指导”2中医气功八段锦、六字诀的主要理论依据之一。

4.调畅情志,戒“焦”戒“躁”

新冠肺炎患者(包括疑似患者),无论病情轻重,均会或多或少地出现一些负面情绪,如焦虑、烦躁、恐惧等,而这些负面情绪都会降低人体的抗病能力。如:《内经》有“勇者气行则已,怯者则着而为病”之论,又有“悲哀愁忧则心动,心动则五脏六腑皆摇”“忧、恐、悲、喜、怒,令不得以其次,故令人有大病”等告诫。中医气功学以此为戒,以“恬惔虚无,真气从之,精神内守,病安从来”为总则,通过“呼吸精气、独立守神、肌肉若一”的锻炼,在强壮形体的同时,转移患者的注意力,调整气机,纾解情志,改善或克服不良情绪。如三线放松功,应用“长呼短吸”、呼气时默念“松”字音,并自上而下意守身体的一个部位,实现放松肢体、调控情绪的目的;再如六字诀,通过呼气时分别发“嘘、呵、呼、呬、吹、嘻”六个不同的汉字,在祛除病邪、调整脏腑的同时,将锻炼者的注意力集中在吐字发音的准确性和呼气时相的延长上,从而起到对负面情绪的改善作用。

5.协同增效,提升“药”力

与药物疗法等常规的“给予性”医疗方法不同,气功疗法是一种通过发掘患者自身抗病潜能的“主动性”医疗方法,具有整体作用、双向作用、协同作用等主要特点。在疫病防治中,最有意义的是其协同作用。所谓协同作用,是指气功疗法与其他治疗方法同时应用时,不但不矛盾,而且还有相互促进、增强疗效的作用,也就是说气功疗法与其他中西医疗法的合理同时应用,可以获得类似“1+1>2”的疗效。大家都已从媒体报道中知道,对这次新型冠状病毒肺炎的治疗,各版《诊疗指南》均提出了包括支持疗法、氧疗、抗病毒治疗、中医药治疗等多种综合性的方法。气功疗法与上述这些中西医疗法之间,都有着良好的协同作用,如若同用可以提升这些广义“药物”的效力。以氧疗为例,氧疗的目的是针对组织供氧不足或利用障碍等缺氧状态,通过给予吸入高浓度氧,使血中溶解氧量增加从而改善组织的供氧;气功锻炼通过调息、调心、调身的有机结合直至最终“融一”,同样可以调整机体的呼吸(包括内呼吸)状态、改善血氧含量。新冠肺炎的中医疗法,国家有关部门2的治疗方案,总体上以驱邪为主;而气功扶正的作用,恰可以与其互补,达到扶正祛邪,协同增效的作用。同时,诸如六字诀之类以强调呼气为主的功法,临床或可助药物驱邪一臂之力。

三、气功战“疫”功法应用

传统中医气功的功法众多,临床应用时应遵循中医辨证施治的原则,但在防治新冠肺炎中也应灵活运用。以笔者之见,重在辨虚实(辨别疾病的虚实)、辨病症(除了辨别疾病属于哪一期之外,还要辨别施治的对象是以解决躯体疾病为主,还是以解决心理问题为主)、辨体质(辨别患者体质的强弱)的“三辨”。大凡病变为实证、患者体质强壮者,可施以动功为主,如以八段锦、五禽戏、五行掌类为主,配以六字诀;反之,对于体质虚弱,或处于恢复期症见明显虚证,乏力明显者,则宜施以坐式或卧式的静功为主,如静坐、内养功、松通养心功等。对于焦虑、烦躁等不良情绪明显者,可施以三线放松功、站桩功等。同时,还要注意因地制宜,根据所处环境及当地的功法资源,选择一定的功法予以锻炼。应当说明的是,虽然气功疗法具有整体性的特点,每种功法对应的疾病相对而言比较宽泛,但是在此次疫情中,也出现了一些有针对性的提升肺气的功法组合,靶向性更强一些,体现了专病专功的特点,平素肺气虚弱的患者可以适当练习,未病先防。如用于治疗,还是建议在有气功治疗经验的医师指导下进行。

四、气功战“疫”适应人群

气功对于疫病防治而言,目前总体处于辅助地位,但其适应人群却很广,除了健康人群与宅在家的易感人群用于自我锻炼、进行主动防护以外,大致可包括以下三类。

1.医学观察期的患者

一般来说,此类人群既有发热、咳嗽等躯体病症,又有恐惧、担心等不良心理,由于此类人群的观察期往往不会太长,故对其以教授简单功法即可,也不必对其操作的准确性提出更高的要求。

2.临床治疗期的患者

大凡此类人群都同时被躯体症状与心理担忧所困扰,而且住院隔离也会有一段时间,对此,以教授一个不太复杂的动功和一个静功为宜,动功如八段锦、五禽戏,静功如放松功、站桩功类。

3.出院恢复期的患者

此类人群的恢复期或许会较长,他们也会或多或少地出现一些不适,同时,还会有担心是否有后遗症等心理问题。因此,对于他们的气功锻炼应该有比较高的要求,包括练功的准确性、每天练功的时间。至于具体功法,如果在临床治疗期间已经教授过功法锻炼的,可以继续锻炼;如果没有接受过气功的话,可以参照“中医康复指导”中2的八段锦、六字诀锻炼,但六字诀总体偏泻,体虚乏力患者一般不适合练习。最好在精通气功疗法的医师指导下学练,并由这些医师负责跟踪指导。

总之,气功疗法是中医传统疗法之一,以此应用于疫病防治的历史悠久,科学的气功锻炼可以起到补益正气、调和阴阳、疏通经络、调畅情志、协同增效等作用,值得在当前新冠肺炎的预防、治疗、康复等各个环节中推广应用。

article------>{"id":668,"cid":1,"sub_cid":"","title":"中医气功战“疫”:提升正气铸盾","short_title":"0","tag_id":"","attr":"1","seo_title":"0","seo_keywords":"","seo_description":"1","source":"网络","author":"admin","description":"新春伊始,面对新冠肺炎的蔓延,一场中西并重、防控结合的疫病阻击战正在如火如荼地进行着。其间,中医药的疗效令人满意:中药复方可改善患者症状、控制病情进展、减轻并发症发生;八段锦、五禽戏等传统中医气功,也出现在湖北武汉一线病房、方舱医院等处,由于其身心并调的特点,受到许多患者的欢迎。","img":"","content":"<p>新春伊始,面对新冠肺炎的蔓延,一场中西并重、防控结合的疫病阻击战正在如火如荼地进行着。其间,中医药的疗效令人满意:中药复方可改善患者症状、控制病情进展、减轻并发症发生;八段锦、五禽戏等传统中医气功,也出现在湖北武汉一线病房、方舱医院等处,由于其身心并调的特点,受到许多患者的欢迎。近日,国家卫生健康委员会办公厅、国家中医药管理局办公室颁发的《新型冠状病毒肺炎恢复期中医康复指导建议(试行)》(以下简称“中医康复指导”),也将八段锦、六字诀分别作为“传统功法”和“其他疗法”予以2。由此,传统中医气功战“疫”再次引起了人们的关注。</p><p><b>一、气功战“疫”文献复习</b></p><p>回顾中医气功学发展的历史,可以清楚地看到,其间不乏气功参与疫病防治的经历,无论是对疫病的预防、治疗,还是康复,古代中医气功学家都留下了十分经典的论述和案例。</p><p>在预防方面,《内经》的论述影响广泛。《素问·遗篇·刺法论》明确指出:“黄帝曰:余闻五疫之至,皆相梁易,无问大小,病状相似,不施救疗,如何可得不相移易者?岐伯曰:不相染者,正气存内,邪不可干,避其毒气,天牝从来,复得其往,气出于脑,即不邪干。气出于脑,即室先想心如日,欲将入于疫室,先想青气自肝而出,左行于东,化作林木;次想白气自肺而出,右行于西,化作戈甲;次想赤气自心而出,南行于上,化作焰明;次想黑气自肾而出,北行于下,化作水;次想黄气自脾而出,存于中央,化作土。五气护身之毕,以想头上如北斗之煌煌,然后可入于疫室。”中医学界乃至广大百姓耳熟能详的“正气存内,邪不可干”这句话,即是古人针对疫病的预防并结合中医气功提出的。其中的操作方法属于气功学中的存想法,现代所说的冥想与此几近相似。</p><p>在治疗方面,最早的记载可能是隋代太医令巢元方主持编写的《诸病原候论》。该书在“时气病诸候·时气候”中,对“病无长少,率相似者”的时气病提出了以肢体运动(调身)为主的导引方:“清旦初起,以左右手交互从头上挽两耳,举;又引鬓发;即面气流通,令头不白,耳不聋。又,摩手掌令热,以摩面从上下二七止。去汗气,令面有光。又,摩手令热,从体上下,名曰干浴。”该书还有针对“温病诸候·温病候”的以存想(调心)为主的导引法:“常以鸡鸣时,存心念四海神名三遍,辟百邪止鬼,令人不病。东海神名阿明,南海神名祝融,西海神名巨乘,北海神名禺强。又云:存念心气赤,肝气青,肺气白,脾气黄,肾气黑,出周其身,又兼辟邪鬼。欲辟却众邪百鬼,常存心为炎火如斗,煌煌光明,则百邪不敢干之。可以入温疫之中。”在“疫疠病诸候·疫疠病候”中也有类似的导引法。</p><p>在康复方面,温病大家叶天士则将气功与药物结合,应用于热病(包括疫病)的康复期。《临证指南医案》卷一“虚劳”、卷二“吐血”中有三个堪称经典的病例。如:对“冷痰上升,交子后干咳,肾虚阳不潜伏”的程姓患者,叶氏医嘱“从摄固引导,勿骤进温热燥药”;而对“阳虚生外寒,阴虚生内热”的何姓患者,叶氏提出的治疗方案是,在用药的同时要求患者“潜心涤虑,勿扰情志,再于子午参以静功,俾水火交、阴阳偶,是药饵以外工夫,皆培植生气之助”;对于“精气暗损为病”的陆姓患者,叶氏认为,此“非草木攻涤可却,山林寂静兼用元功,经年按法,使阴阳渐交而生生自振。徒求诸医药,恐未必有当”。叶氏所倡导的药物疗法与气功疗法相结合、医生处方治疗与患者主动锻炼相结合的康复思路,由此可见一斑。</p><p><b>二、气功战“疫”机理概说</b></p><p>中医气功疗法是一种发挥患者自身抗病潜能的自我身心疗法,其长于扶正而逊于驱邪,由此,对疫病预防、治疗、康复的总以培育正气、铸就盾牌为主,而以因势利导、驱邪外出为辅。具体地说,体现在以下几个方面。</p><p><b>1.补益正气,扶正祛邪</b></p><p>气是中医学的核心概念,大家耳熟能详的“人活一口气”正是道出了中医对人体之气的重视。正如国医大师陆广莘所指出的,中医学十分重视人体的正气,以及由其产生的主体性反应,在治疗中善于发现、依靠、调节人的主体性反应,养生、治病都着眼于“疏其血气,令其条达,而致和平”(《素问·至真要大论》),即扶助正气以实现祛除病邪,恢复健康状态。中医气功正是通过自我主动锻炼身形、调节呼吸、调整心理思维与情绪活动,着眼于补益人体的正气,通俗说也就是提升正能量,建立形、气、神之间的正向循环,提高人体的自我抗病、自我修复、自我疗愈、自我调节能力,重在调动人体内在的积极性主动性,到达扶正祛邪,甚至是不战而屈人之兵的作用,最终到达神安、气充、形健,实现“正气存内,邪不可干”的健康目的。八段锦、五禽戏、六字诀等中医气功代表性功法在此次抗击新冠肺炎疫情中的应用,正是基于这一机理,激发人体自身正气,鼓荡患者气血,鼓舞必胜信心,最终战胜疫情。</p><p><b>2.调和阴阳,权衡以平</b></p><p>“和”是人体健康的重要保证,也是健康的标志,包括人与自然的和谐、人体内部功能的和谐。上海市名中医王庆其先生认为“健康是一种‘三和’的状态”健康的本质就是“天人和”(人应该顺应自然界阴阳变化)、“形神和”(做好生理和心理的调摄,保持心身协调)、“气血和”(通过各种措施使人体气血充沛运行有序)。笔者研究气功学中的“和”,得出的观点与王先生不谋而合,发现气功锻炼能够起到顺天辟邪和天人、动静结合和形神、辨证施功和阴阳、调畅气机和内气、相辅相成和脏腑等“和”的作用,这在气功战“疫”中具有不小的意义。以“中医康复指导”2的八段锦为例,其中的第一段“两手托天理三焦”、第三段“调理脾胃需单举”的主要作用是和谐气机的升降;第二段“左右开弓似射雕”(旧称“左肝右肺似射雕)、第五段“摇头摆尾去心火”的作用是和谐脏腑、水火与五行生克。再以其所2的六字诀为例,该功法强调呼气发音,旨在将人体不和之气与从外界吸入的疫病之气“吐”出体外,即古人所说的将“我之气还天地”。</p><p><b>3.疏通经络,因势利导</b></p><p>经络于人体健康和疾病防治的重要性是不言而喻的。李时珍“医不知此,罔探病机;仙不知此,难安炉鼎”之论,尽管讲的是奇经八脉的重要性,但实际也是古代医家对整个经络系统重要性的经典论述。气功,古代也有“导引”之称,在《黄帝内经》时代,“导引按蹻”与“毒药”等并列为当时最主要的五大医疗措施之一,故当代历史学家郭沫若先生说:“古人所说的道(导)引,即今人所说的气功。”导引在中医气功学中,又是一个动词,义近“引导”。气功锻炼时,通过形体的运动(调身)、呼吸的调控(调息)、身心的安静和意念的应用(调心),来导引内气的运行(包括运行的方向和速度),由此充实脏腑之气、活跃经络之气,并以此实现防病治病的目的。具体地说:在未患病或患病的早期施以气功,可奏畅气血、利九窍之效,诚如张仲景之所谓“若人能养慎,不令邪风干忤经络。适中经络,未流传脏腑,即医治之。四肢才觉重滞,即导引,吐纳,针灸,膏摩,勿令九窍闭塞”。金元四大家之一的张从正,还将华佗五禽戏等导引列为汗法而用于临床。这些,对新冠肺炎疑似患者和早期患者的治疗颇有参考价值。在患病以后和康复阶段,科学合理的气功锻炼,可以畅通经络,因势利导驱邪外出,这或许就是“中医康复指导”2中医气功八段锦、六字诀的主要理论依据之一。</p><p><b>4.调畅情志,戒“焦”戒“躁”</b></p><p>新冠肺炎患者(包括疑似患者),无论病情轻重,均会或多或少地出现一些负面情绪,如焦虑、烦躁、恐惧等,而这些负面情绪都会降低人体的抗病能力。如:《内经》有“勇者气行则已,怯者则着而为病”之论,又有“悲哀愁忧则心动,心动则五脏六腑皆摇”“忧、恐、悲、喜、怒,令不得以其次,故令人有大病”等告诫。中医气功学以此为戒,以“恬惔虚无,真气从之,精神内守,病安从来”为总则,通过“呼吸精气、独立守神、肌肉若一”的锻炼,在强壮形体的同时,转移患者的注意力,调整气机,纾解情志,改善或克服不良情绪。如三线放松功,应用“长呼短吸”、呼气时默念“松”字音,并自上而下意守身体的一个部位,实现放松肢体、调控情绪的目的;再如六字诀,通过呼气时分别发“嘘、呵、呼、呬、吹、嘻”六个不同的汉字,在祛除病邪、调整脏腑的同时,将锻炼者的注意力集中在吐字发音的准确性和呼气时相的延长上,从而起到对负面情绪的改善作用。</p><p><b>5.协同增效,提升“药”力</b></p><p>与药物疗法等常规的“给予性”医疗方法不同,气功疗法是一种通过发掘患者自身抗病潜能的“主动性”医疗方法,具有整体作用、双向作用、协同作用等主要特点。在疫病防治中,最有意义的是其协同作用。所谓协同作用,是指气功疗法与其他治疗方法同时应用时,不但不矛盾,而且还有相互促进、增强疗效的作用,也就是说气功疗法与其他中西医疗法的合理同时应用,可以获得类似“1+1>2”的疗效。大家都已从媒体报道中知道,对这次新型冠状病毒肺炎的治疗,各版《诊疗指南》均提出了包括支持疗法、氧疗、抗病毒治疗、中医药治疗等多种综合性的方法。气功疗法与上述这些中西医疗法之间,都有着良好的协同作用,如若同用可以提升这些广义“药物”的效力。以氧疗为例,氧疗的目的是针对组织供氧不足或利用障碍等缺氧状态,通过给予吸入高浓度氧,使血中溶解氧量增加从而改善组织的供氧;气功锻炼通过调息、调心、调身的有机结合直至最终“融一”,同样可以调整机体的呼吸(包括内呼吸)状态、改善血氧含量。新冠肺炎的中医疗法,国家有关部门2的治疗方案,总体上以驱邪为主;而气功扶正的作用,恰可以与其互补,达到扶正祛邪,协同增效的作用。同时,诸如六字诀之类以强调呼气为主的功法,临床或可助药物驱邪一臂之力。</p><p><b>三、气功战“疫”功法应用</b></p><p>传统中医气功的功法众多,临床应用时应遵循中医辨证施治的原则,但在防治新冠肺炎中也应灵活运用。以笔者之见,重在辨虚实(辨别疾病的虚实)、辨病症(除了辨别疾病属于哪一期之外,还要辨别施治的对象是以解决躯体疾病为主,还是以解决心理问题为主)、辨体质(辨别患者体质的强弱)的“三辨”。大凡病变为实证、患者体质强壮者,可施以动功为主,如以八段锦、五禽戏、五行掌类为主,配以六字诀;反之,对于体质虚弱,或处于恢复期症见明显虚证,乏力明显者,则宜施以坐式或卧式的静功为主,如静坐、内养功、松通养心功等。对于焦虑、烦躁等不良情绪明显者,可施以三线放松功、站桩功等。同时,还要注意因地制宜,根据所处环境及当地的功法资源,选择一定的功法予以锻炼。应当说明的是,虽然气功疗法具有整体性的特点,每种功法对应的疾病相对而言比较宽泛,但是在此次疫情中,也出现了一些有针对性的提升肺气的功法组合,靶向性更强一些,体现了专病专功的特点,平素肺气虚弱的患者可以适当练习,未病先防。如用于治疗,还是建议在有气功治疗经验的医师指导下进行。</p><p><b>四、气功战“疫”适应人群</b></p><p>气功对于疫病防治而言,目前总体处于辅助地位,但其适应人群却很广,除了健康人群与宅在家的易感人群用于自我锻炼、进行主动防护以外,大致可包括以下三类。</p><p><b>1.医学观察期的患者</b></p><p>一般来说,此类人群既有发热、咳嗽等躯体病症,又有恐惧、担心等不良心理,由于此类人群的观察期往往不会太长,故对其以教授简单功法即可,也不必对其操作的准确性提出更高的要求。</p><p><b>2.临床治疗期的患者</b></p><p>大凡此类人群都同时被躯体症状与心理担忧所困扰,而且住院隔离也会有一段时间,对此,以教授一个不太复杂的动功和一个静功为宜,动功如八段锦、五禽戏,静功如放松功、站桩功类。</p><p><b>3.出院恢复期的患者</b></p><p>此类人群的恢复期或许会较长,他们也会或多或少地出现一些不适,同时,还会有担心是否有后遗症等心理问题。因此,对于他们的气功锻炼应该有比较高的要求,包括练功的准确性、每天练功的时间。至于具体功法,如果在临床治疗期间已经教授过功法锻炼的,可以继续锻炼;如果没有接受过气功的话,可以参照“中医康复指导”中2的八段锦、六字诀锻炼,但六字诀总体偏泻,体虚乏力患者一般不适合练习。最好在精通气功疗法的医师指导下学练,并由这些医师负责跟踪指导。</p><p>总之,气功疗法是中医传统疗法之一,以此应用于疫病防治的历史悠久,科学的气功锻炼可以起到补益正气、调和阴阳、疏通经络、调畅情志、协同增效等作用,值得在当前新冠肺炎的预防、治疗、康复等各个环节中推广应用。</p><p>作者简介</p><p>黄
